The mark 1 EMH from the USS Equinox. When the Equinox crew decided use the Nucleogenic Lifeforms to enhance their warp drive they deleted the EMH's ethical subroutines to make him more amenable to helping with the work. [1]

When Voyagers EMH visited the Equinox to investigate what the crew had been up to. The Equinox EMH trapped him here and stole his mobile emitter. Returning to Voyager to pose as their EMH. He was able to free the Equinox crew and allow them to scape back to their ship. [1]
