Though there may have been other wars in individual countries (like a civil war in China or India).
And we have very little information on the Eugenics wars (exept that it should have happened by now  ).
Man, you'd have to kill alot of people for America to be contributing most of the personnel to Starfleet (assuming that everywhere outside the West in ST isn't the shithole it is today). Alot of people in China and India live in rural areas, so I imagine that for this to work they would resemble a glass parking lot. Or there was a ridiculous amount of chemical or biological weapons used, which would still render most of those areas unihabitable.

Didn't ENT retcon the Eugenics Wars into WWIII?

I don't think they even mentioned it.

I know there was an episode of ENT that dealt with the Eugenics Wars (although I didn't see it - I stopped watching after the second season), but I don't think they specified when they occurred. But it would have been cool if had made more of an effort to do that. For example, the Eastern Coalition that Lily mentioned in First Contact would have fit perfectly as a coalition of nations led by Khan.

Well, the Augments arc of season 4 covered the genetic augmentation aspect, but I don't remember them giving any specifics about the war itself.

Apparently it was mentioned in "Borderland".

I haven't seen any Enterprise since it was on the air, so my memory is a bit hazy when it comes to the details. :?

The united states may not have even been involved in WWIII as a war zone, considering the 1996 flashback of the milennium gate, especially if you pair WWIII up with the eugenics wars. Also, the milennium gate shows that the US still had sufficient amounts of money for such a huge project.

In 2004, ENT crewmembers temporarily visited carpenter street. Sober, but seemingly not affected by war. Then again, these places looked very prospering, perhaps due to putting away the problems, sanctuary districts avant la lettre?

I liked it! I really did, especially after they picked up the tempo. But I friggin LOVED when they did the alternate version for the Terran Empire. That was awesome.

One thing about Crusher... she's from the Moon. IIRC, her bio listed it as her lace of birth in the episode where they lost their memories and got roped into the war with the Satarrans.
