Lets not forget minor farkups that could have cost lives, such as letting Harry return to the generational ship unguarded in "The Disease" comes to most recent memory. But it's a multi-times a season thing. Did Picard/Sisko ever make mistakes like that? We'll discount Kirk, as his beaming down to every planet with McCoy, Spock, and Ensign Ricky nearly tops Janeway.
