Well I suppose there's no place better to ask than here. I don't have any Photoshop skills and wouldn't attempt to create such an image myself. The image is just a silly thing I've imagined by flipping the title of "The trouble with tribbles" to "The Tribble with Troubles;" depicted as a tribble on the therapist's couch.
