Surely he was just using "door" as a colloquialism for "doorway." In much of America, "door" is used both as door as to which you refer and as "doorway."

I remembered "...don't call me Shirley" but not the "serious" half of the joke. Though I came thisclose to posting something about it anyway, I just figured it was a bit of an old joke.
