I thought she was one of his senior staff. She'd served with him before she told Tom, and assuming Voyager was her first command................

At least I HOPE it was her first command. (Somebody told me that in a novel she had a ship before Voyager...scary)

Maybe she'd never fucked up big time before? I can sort of see her being a kind of "stealth failure". Someone who works their way up through the ranks giving fine ideas and displaying some level of competence....only to get to a certain point where they throw sanity out the window.

Voyager was NOT Janeway's first command.

In Revulsion she described how Tuvok was her tactical officer on her first command, and openly ciritcised her in front of three Starfleet Admirals for neglecting tactical procedures on the ship in favour of sciences.

Interesting. That may very well be what prompted her assignment as captain of Voyager.
