I thought she was one of his senior staff. She'd served with him before she told Tom, and assuming Voyager was her first command................

At least I HOPE it was her first command. (Somebody told me that in a novel she had a ship before Voyager...scary)

Maybe she'd never fucked up big time before? I can sort of see her being a kind of "stealth failure". Voyager was NOT Janeway's first command.
In Revulsion she described how Tuvok was her tactical officer on her first command, and openly ciritcised her in front of three Starfleet Admirals for neglecting tactical procedures on the ship in favour of sciences.
