IIRC, it was one of TSR's games sold as a box set - rulebook, accessories, and initial campaign adventure. Based strongly on Bond films, the James Coburn Flint movies, Dean Martin's Matt Helm movies, etc. Weird combat system, involving cross-referencing to find results on a table. And I think it was pre-'90's.

Ah! here we are. 1980.

Yep, the combat system left something to be desired. I remember the big russian character I mentioned before had a high constitution and a toughness advantage, so he could take 10 damage boxes to each body part. He took a shotgun blast at close range to the head, and not only survived, but kept fighting.......................................................
